MED 8421 - Patient Student Partnership 2A

Students address patient vulnerability during transitions of care and the complexity of managing chronic disease complications. Students will be expected at least monthly to try to attend some of their patients’ primary care and specialty visits, maintain contact through telephone and meetings, visit hospitalized patients, and help advocate for their patients’ health and social needs.
